Dental Plan

Good dental care is an important part of maintaining your overall health. The Suiza Dental Plan offers three dental options to cover preventive care, basic care, and major care. The options vary in the amount paid for covered services, deductibles, and annual maximum benefits With all three options, you may visit any dentist or specialist. However, when you use a network dentist, you will not have to complete claim forms and you pay only your share of the cost when you receive care. You also can be sure that a network dentist won't charge you more than the amount the plan will pay for a specific service (the reasonable and customary limit) If you visit a non-network dentist, you may have to complete your own claim forms and you are responsible for paying any difference between the plan's reasonable and customary amount and the dentist's charge. The Suiza Dental Plan is administered by Delta Dental Insurance Company.
